Depending on how good of a grip you might have you can use the three parts of the hand separately or altogether. For example if you are massaging a persons small of the back it's usually easier to only use the thumbs instead of the rest of the fingers. For a person's middle back about heart level it's usually simpler to just press your palms in their back making a slow circling motion. Do whatever feels comfortable for yourself.

Simply move your hand to your opposite shoulder, lay the palm of your hand at the top of your shoulder and extend your fingers down at an angle towards your spine.
